    Do you have any recommendations of getting insurance or medicare to pay for a dietition? I got one from stanford in Pleasanton, CA and they were recommending advice from the 80's like white eat rice and white bread.

    Hi Doctor, I am a kidney recipient and I have completed 7 years since my kidney transplant. For the past 3 years, I have been experiencing fatigue and difficulty in breathing. I have mentioned this to my doctor, but he does not seem to be focusing on it. My other reports, including brain MRI, echo-cardio, EKG, and my kidney function test, are normal, except my serum bicarbonate level, which is 21.9, and urine pH, which is 5. The urine pH is decreasing each year, and I am feeling imbalanced while walking. I am not able to walk properly and feel very tired all the time. What could be the reason for this, and how can I overcome it? By telling again and again he started sodium bicarbonate 1000mg thrice a day. Please let me know if my treatment is going well, or if I need to change my nephrologist. It will be a great help if you give me some advice. Thank you

    This is such a complex problem to study, because there’s just so many variables and so much variance among individual athletes. While endurance athletes are more likely to have higher CAC scores it’s well established that those CAC scores don’t quite translate into same risk as for non-athletes. It’s likely that some athletes are more prone (genetically) to build plaque and high intensity endurance exercise disturbs the blood flow in a way that’s damaging to the inner wall of coronary vessels, but it’s also been shown that 8+ hours / week of cardiovascular exercise can significantly improve collateral blood flow as well as diameter of vessels - there are all these healthy adaptations (calcification of plaque included) that seems to be protective for many masters runners. Still, I think that taking pre-race aspirin to reduce a risk of transiently elevated risk of cardiac arrest (usually in the final miles of marathon) is a good idea for middle aged runners who are at intermediate to high risk. Also, some people may benefit from statin or other therapies…

    If the test adding caffeine showed better results, common wisdom would indicate that caffeine is the good guy. Has anyone ever tested tea drinkers? Asians, noted consumers of tea, are well known as testing higher intellectually than average. I am quite intrigued, and I drink hot tea daily.

    Hello doc, my doctor started me on sglt2 jardianne, first hand tab I started I develop excruciating itchiness, I stopped it and let him know he ordered stop. What are other SGLT2 medications Doc, thanks

      Its important to understand if you are allergic to SGLT2 as a class. If so, then we wouldn't use any of them. Talk to your doctor and see if its worth trying one more time, at a lower dose.

    I suspect we would be better off just using a little sugar than any of the artificial sweeteners. A teaspoon of sugar on your oatmeal in the morning probably won't do much damage to your health, we run into problems when we are consuming the Big Gulp soft drinks every day with 32 teaspoons of sugar in each and having 5 donuts for breakfast.

    Coffee wakes you up in morn. It has that feel good factor as well. The problem for sure is that most of these "take away" places specialize in the "exotic" coffee, usually hideously over priced. Within these, lies your true enemy : namely sugar and additives. Be careful....

    Dr Hashmi, I sent your link to my brother with stage 3 CKD. I hope he will follow your advice. I find Filipinos have CKD conditions because their diet is based on the American diet. I am a Filipina from England. I don't have any debilitating diseases except I have osteopenia and congenital lung (right lung only one lobe instead of three). At 79, I am muscular, very lean and have smooth skin. I cook my food from basic ingredients. Very seldom do I eat processed foods. Occasionally, I buy prepacked quiche. I avoid eating processed meats.

    Coffee decreases insulin sensitivity mainly after eating …. Coffee is strong acidic for blood 😢 so it’s not healthy at all

    So that's where the normal range is different for men and for women, and my doctor isn't worried about me, as a 5'11"woman who walks with a heavy backpack a lot etc., having a creatine level over the female range but within the male range. Thank you!
